Welcome back to our space, Mending Healthcare has established a relationship with you. Follow-up appointments are very important to ensure we follow through and are successful in your care, your needs and getting your life back. Follow up appointments consist of any changes in behavior, symptoms, and side effects to medication. Any new changes in behavior or symptoms you may experience. We want to know improvement in health, any concerns you may have. We want to know if you are compliant with care, treatment plan, and medications if you were given any. We will monitor your care, treatment plan, and efficacy of medications. We want to monitor when to adjust your treatment plan. We will follow up with our clients ranging from 1 week to 2 months based on your needs. We encourage you to follow up with all your appointments and as needed for a better optimum outcome.
How to Prepare
Taking some time to prepare before your upcoming appointment with your psychiatrist is valuable for maximizing its benefits. Here are some things to consider:
- Create a list of new significant changes or issues you wish to discuss with your mental health provider.
- Take note of any new symptoms or changes in existing or prior symptoms. We would like you to write down your symptoms on a daily basis to enable tracking changes, progress and impact in your quality of life.
- Document any changes in your overall physical health.
- Include information about your family’s history of physical and mental health conditions you remember that was not noted on your first visit.
- Gather feedback on how others perceive your well-being, particularly from family, friends, and co-workers who care about you.
- Bring a list of all the current medications and substances you take and any new medication/substance you forgot on your first visit. Include prescription medications, over-the-counter medications, vitamins, supplements, herbal products, and any recreational drugs.
